Family Physician
Family Medicine is a medical specialty focused on providing comprehensive health care to people;
Family Doctors provide a range of acute, chronic, and preventive medical care services. In addition to diagnosing and treating illness, they also provide preventive care, including routine checkups, health-risk assessments, screening tests, and personalized counselling on maintaining a healthy lifestyle. Family physicians also manage chronic illness, often coordinating care provided by other subspecialists.
Primary Care Nurse
Primary Care Nurses support family physicians in improving primary care through relationships, collaboration, and innovation and work with other health professionals to coordinate the comprehensive delivery of primary care services for their patients. This includes longitudinal follow-up for chronic diseases, individualized health education, and ensuring proper handover from hospital discharges.
